This transition is particularly important as they turn 18, when many young adults move out for college, enter the workforce, or begin living independently. Without structured guidance, adjusting to self-care, navigating the healthcare system, and advocating for medical accommodations can be overwhelming. The Adolescent Transition Module serves as a strategic framework to prepare adolescents for this shift, equipping them with the tools to manage their health confidently and ensuring continuity of care into adulthood.

By preparing them early, we ensure that they can confidently manage their condition, secure necessary accommodations for school or work, and live fulfilling, independent lives.
