PRIMARY GOAL: Ensure all aspects of the transition plan are addressed in a timely manner.

Key Steps:

  • 6-12 Months Before Transition

  • Begin identifying adult care providers.
  • Start reviewing insurance options.
  • Begin discussing educational or vocational plans.

Example: Sarah’s care team organizes a meeting with her, her parents, and the adult hematologist to discuss the transition process and set a timeline.

  • 3-6 Months Before Transition

  • Ensure medical records are transferred.
  • Confirm the adolescent understands medication management.
  • Finalize the college/work accommodation plan if applicable.

Example: Sarah meets with her high school counselor to finalize her post-graduation plans, including college accommodations.

  • Post-Transition (1-3 Months After Transition)

  • Follow up to ensure the adolescent has successfully established care with an adult provider and is managing their condition independently.
  • Revisit any unmet psychosocial, educational, or financial needs.
