To be eligible for burial assistance, the adult and/or child client must have had sickle cell disease and had received care in Davidson County, Shelby County, Knox County or Hamilton County, Tennessee.

• These funds shall be used by the family/guardian as the last resort; therefore, Sickle Cell clients family/guardian must have exhausted other community/family resources prior to receiving benevolent assistance through Sickle Cell Foundation of Tennessee (SCFT).
• A Sickle Cell client’s family/guardian can receive a maximum of $500.00. Funds are distributed on a first come first serve basis and paid directly to the Funeral home.
